What Is Gastrointestinal Bleeding?
The gastrointestinal tract includes the esophagus, stomach, small intestine, large intestine, rectum, and anus. According to the National Institute of Diabetes and Digestive and Kidney Diseases, GI bleeding may be acute or chronic and may occur in the upper, middle, or lower digestive tract.
Acute GI bleeding begins suddenly. It may range from limited bleeding to severe haemorrhage with shock.
Chronic GI bleeding occurs slowly, persists over time, or repeatedly stops and starts. The blood may be too small in quantity to see without laboratory testing.

Hematemesis means vomiting fresh red blood. It usually indicates bleeding from the esophagus, stomach, or duodenum and requires urgent medical assessment.
Coffee-Ground Vomit
Dark vomit resembling coffee grounds contains blood that has been partly altered by stomach acid. It suggests an upper GI source, even when active vomiting of fresh blood has stopped.
Melena
Melena is black, tarry, sticky, and often strong-smelling stool caused by digested blood. It commonly indicates an upper GI bleed, although small-bowel or right-sided colon bleeding may also cause it.

Occult GI Bleeding

Common Causes of Upper GI Bleeding
Peptic Ulcer Disease
Peptic ulcers are open sores in the stomach or duodenal lining. Helicobacter pylori infection and nonsteroidal anti-inflammatory drugs, including ibuprofen, naproxen, and aspirin, are the leading causes.
Esophageal or Gastric Varices
Varices are enlarged veins that develop because of increased pressure in the portal venous system, often due to advanced liver disease. Ruptured varices may cause sudden, severe, and life-threatening bleeding.
Gastritis and Esophagitis
Inflammation or damage to the stomach or esophageal lining may produce erosions and bleeding. Possible contributors include H. pylori, alcohol, NSAIDs, severe illness, and acid reflux.
Mallory-Weiss Tear
Forceful or repeated vomiting may tear the lining near the junction of the esophagus and stomach. This injury may cause fresh blood in vomit.
Common Causes of Lower GI Bleeding
Diverticular Bleeding
Diverticula are small pouches in the colon wall. A damaged blood vessel inside a pouch may cause sudden, painless, and sometimes heavy rectal bleeding.
Hemorrhoids and Anal Fissures
Hemorrhoids may produce bright red blood on toilet paper, in the toilet, or on the stool surface. Anal fissures commonly cause bleeding with sharp pain during or after bowel movements.
Visible blood should not automatically be blamed on hemorrhoids. Polyps, inflammatory bowel disease, and colorectal cancer may produce similar signs.
Colitis and Inflammatory Bowel Disease
Ulcerative colitis and Crohn’s disease may cause bloody diarrhoea, urgency, abdominal pain, fever, and weight loss. Infectious or ischemic colitis may also lead to bleeding.
Polyps and Colorectal Cancer
Colon polyps and colorectal cancer may cause visible bleeding or slow, hidden blood loss. Other possible signs include altered bowel habits, unexplained weight loss, abdominal discomfort, or iron-deficiency anemia.

Initial Clinical Assessment

Blood Tests
A complete blood count measures haemoglobin, hematocrit, and platelets. Other tests may assess kidney and liver function, clotting, blood group, and compatibility for transfusion.
Haemoglobin may initially appear normal after sudden blood loss because plasma and red blood cells are lost together. Repeated measurements and clinical observations are often more useful than one result.
Stool Testing
Stool guaiac or faecal immunochemical testing may detect hidden blood. These tests are not designed to confirm an obvious acute bleed when a patient already has hematemesis, melena, or visible rectal blood.
A positive occult blood test also does not identify the bleeding source. Further investigation is usually required.
Endoscopy and Colonoscopy
Upper GI endoscopy examines the esophagus, stomach, and duodenum. It may locate and treat ulcers, varices, tears, and vascular abnormalities.
Colonoscopy examines the colon and rectum. It helps identify diverticular bleeding, colitis, polyps, cancer, and vascular lesions. The 2023 ACG guidance supports nonemergency inpatient colonoscopy for most hospitalised lower GI bleeds because urgent colonoscopy within 24 hours has not consistently improved major outcomes.
CT Angiography and Other Tests
CT angiography may rapidly locate active, significant bleeding. A positive result may guide catheter angiography and embolisation.
Capsule endoscopy or deep enteroscopy may be used when standard upper endoscopy and colonoscopy do not find the source. Capsule endoscopy is generally used after the patient is stable and when intestinal obstruction is not suspected.
Treatment of Gastrointestinal Bleeding
Treatment depends on the bleeding source, severity, vital signs, laboratory results, and other medical conditions.
Initial Stabilisation
Emergency management may include:
- Securing the airway when needed
- Establishing large-bore intravenous access
- Giving intravenous fluids
- Collecting blood for laboratory testing and crossmatching
- Providing oxygen for hypoxemia or respiratory distress
- Transfusing blood when clinically indicated
- Monitoring urine output and mental status
A restrictive red blood cell transfusion threshold near haemoglobin 7 g/dL is used for many hospitalised upper GI bleeding patients. It is not a universal rule. Active massive bleeding, cardiovascular ischemia, symptoms, age, and other conditions influence the decision.
Medicines
Proton pump inhibitors are used for ulcer-related bleeding and selected high-risk endoscopic findings. Suspected variceal bleeding may require vasoactive medicine and antibiotics alongside urgent specialist care.
H. pylori-positive ulcers require eradication treatment. Inflammatory bowel disease, infection, and other causes require condition-specific therapy.
Vitamin K is not a general treatment for every GI bleed. It is used in selected situations, such as clinician-directed reversal of warfarin. Anticoagulant and antiplatelet management depends on bleeding severity, the specific medicine, and the patient’s clotting risk.
Endoscopic Treatment
Endoscopic haemostasis may involve:
• Clips to close or compress a bleeding vessel
• Thermal treatment to seal a vessel
• Injection therapy
• Band ligation for esophageal varices
• Haemostatic powders or other specialised devices
Upper endoscopy is generally performed within 24 hours for hospitalised upper GI bleeding after appropriate stabilisation, according to ACG guidance.
Angiographic Embolisation and Surgery
Interventional radiologists may use angiography to locate and block a bleeding blood vessel. Surgery may be required when bleeding remains uncontrolled, repeatedly returns, or results from perforation, ischemia, or another surgical condition.
Nursing Interventions for GI Bleeding
Nursing care focuses on early recognition, haemodynamic stability, safe treatment, and preparation for procedures.
Key interventions include:
• Monitor vital signs and level of consciousness
• Assess skin temperature, capillary refill, and peripheral perfusion
